Dental ID Cards
After your benefit election in Dayforce becomes effective, Delta Dental will mail you an ID Card along with your Certificate of Coverage and Policy. Show your ID card each time you receive covered services from a provider. Only members on your plan can use your ID card.
If you haven’t received your ID card before getting a covered service, or if you lose it, you can print a temporary card by logging into your Delta Dental member website.
