Coke Florida offers two medical plans designed to help you or your family maintain good health and protection from the financial burden of a serious illness or injury. Deciding which plans are best for you depends on your specific health care needs, provides preferences, budget, and lifestyle. You can select from the options below.

Please note that both plans are administered through Aetna and give you access to the same network of high-quality medical providers. The difference is that each plan carries different premiums and out-of-pocket costs.

The Choice Plus Value Plan has higher premiums but lower out-of-pocket costs.

Deductibles, copays, and coinsurance accumulate toward the out-of-pocket maximums. Usual, Customary, and Reasonable (UCR) charges apply for all out-of-network benefits.

Embedded Deductibles and Out-of-Pocket Maximum:

For Associate-only coverage, the amounts are represented as single coverage.

For family coverage, the plan actually has two deductible amounts. Each covered individual within the family will not exceed the applicable individual amount listed, and the total fmaily deductible is satisfied once 3 family memebers meet the inividual amount. For example, if only one inidivual in the family is receiving medical services, they only have to meet the inidivual deductible and inidivdual out-of-pocket maximim. However, if there are 5 members in the family and 3 memebers meet their inidivudal deductibles, then all family memebers are deemed to have satisfied the family deductible.

If only one individual is receiving medical services, they still have to meet the family deductible and family out-of-pocket maximum. In a family coverage situtaiton, this may put more financial risk for payment of medical bills if only one members of the family is receiving medical services than an embedded deductible.

Aetna will mail you an ID card along with your Certificate of Coverage and Policy after your benefits go into effect. Show your ID card each time you receive covered services from a provider. Only members on your plan can use your ID card.

If you haven't received your ID card before getting a covered service, or if you lose it, you can print a temporary card by logging in to your Aetna member website.
