New Hires

Benefits are effective on the first of the month after 60 days of continuous employment.

Example: If you are hired on January 15th, your benefits are effective April 1st provided you enroll by February 15th.

Be sure to make your benefit elections within 31 days of your hire date; otherwise, your next opportunity to enroll will be during the annual Open Enrollment period.

Step 1: Carefully review this website and become informed about your benefit plans and choices.

Step 2: Be prepared with the names, dates of birth, social security number (and their respective documentation) of the dependents who you plan on enrolling for coverage.

Step 3: Complete your enrollment online using either of the two options listed below. 

Step 4: Confirm your elections and print a copy of your confirmation statement for your records.

 

Visit the Human Resources SharePoint site or scan the QR code. Click the HR tab, and then click HR Self Service (Dayforce) from the drop-down menu. Then follow the prompts to complete your enrollment. 

Schedule an appointment at https://cokeflorida.benefitsinfo.com/schedule-a-call to meet with a Benefits Counselor virtually. These counselors are available to assist you with enrollment and/or answer any questions you may have about benefits. While we hope you take advantage of their help, you are not required to speak to a counselor to complete your enrollment. The counselor can share their screen and will be on camera, but the use of your camera is optional.

For eligible dependents, coverage if elected, begins at the same time your associate coverage beings. You must submit proof that your dependents are eligible to be covered on our plan before we can complete your election or change request. The following supporting documents are required for any dependent:
 

 Spouse

 Children   

 Birth certificate

 Birth certificate

 Social Security Card

 Social Security Card

 Marriage license 

 Legal documents (disabled children over age 26

 

  • The benefits you choose will remain in effect from the date of your eligibility through December 31 of the calendar year. After you enroll, you will be able to make changes to your benefit selections ONLY if you have a qualified life event during the year. Your next opportunity to make changes will be during the annual Open Enrollment for the next plan year.

  • We strongly encourage you to review your benefits elections. If any of the information or benefits are not accurate, please contact HR Services to arrange for changes as soon as possible.

  • If you enroll in any of the following plans, you should expect to receive your ID card prior to your benefits effective date. Please share these cards with your provider and pharmacy on your next visit:

    • Medical (+ Prescription)

    • Dental

    • Health Savings Account

    • Flexible Spending Account

Should you experience a qualifying event that results in the loss of coverage, your benefits will be terminated accordingly:
 

  • Medical, Dental, and Vision Insurance

    • Date of Event: Death, Job Termination

    • End of Month: Divorce, Loss of Dependent Status
       

  • Flexible Spending Accounts (FSA)

    • Date of Event
       

  • Hospital Indemnity Insurance, Accident Insurance, Critical Illness Insurance
